Job Description
JOB PURPOSE AND REPORTING STRUCTURE: Under the direction of the Chief Medical Officer the Dietician supports the primary care provider network with nutritional assessment, education and management of patients with chronic conditions who are at risk for disease progression, complications, cognitive and functional decline. In collaboration with providers and interdisciplinary team, the Dietician develops and implements an individualized care plan designed to promote the patients' understanding and management of their condition, optimize quality of life and improve clinical outcomes. The Dietician conducts nutrition education sessions for individuals/groups using clinical guidelines. Also, develop various nutritional programs to reduce disparities in rates of cancer, diabetes and obesity. The Dietician will screen and support patients in adopting healthy lifestyles. Group and individual services will be delivered by different practice teams focusing on diabetes, a subset of obese children and post-menopausal women.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Conduct comprehensive nutritional assessment, education and management of patients referred by providers or multidisciplinary team.
- Develop and deliver educational programs and appropriate teaching tools for various ethnic and cultural groups to support understanding of nutrition.
- Provide instruction to patients and clients in the use of modified diets to health.
- Coordinate, implement and evaluate nutritional education using national standards and guidelines.
- Provide high- risk and routine nutrition counseling.
- Maintain timely and complete documentation and billing of all care management encounters.
- Relate to people from diverse ethnic and cultural backgrounds.
- Comply with all regulatory standards of federal, state, and accrediting agencies.
- Adhering to HIPAA regulations, and patient rights policies.
- Perform other job-related duties as assigned.

EDUCATION and/or EXPERIENCE: Bachelor's degree in Dietetics and Wisconsin State Licensure and RD Credential is required during the time of employment. One to two years of experience in a clinic or hospital setting is preferred. Possess the necessary skills to effectively utilize Microsoft Office applications, proficiency in the EMR system and nutrient analysis programming is required. Also, proficiency in written, oral and electronic communication is required.
